Appointment Request Form
Delivery is by appointment only. Please use the form below to request an appointment, which will ensure we have the time and bandwidth to process your delivery. Dani (inventory manager) will probably approve your requested time as long as it is on a Monday, Tuesday, or Wednesday between 11 and 5.
Please do not come to the store before we confirm your appointment.
